Waiver of Liability for class participation and individual appointments.

WAIVER OF LIABILITY

I understand that I am participating in group classes including but not limited to yoga and I will be receiving instructions, which may include strenuous physical activity. I understand that these classes are not a substitute replacement for medical care. I take full responsibility for my own well-being including my physical, mental, and emotional health.

I release Blue Moon Apothecary & Wellness Studio and Blue Moon Yoga & Movement Studio from any and all injuries or damage that may occur from my participation in these classes, training, workshops, physical or otherwise. I also understand that photos/videos may be taken and utilized for marketing purposes. I waive any and all claims against these businesses, any teacher or facility owner, including Blue Moon Collective LLC.

Cancellation Policy

CANCELLATION POLICY (Appointments)

I understand that any cancellations for individual appointments are required to be requested ASAP. Any cancellations requested with less than a 24-hour notice will not be granted, and I will be charged for the cost of the appointment.

CANCELLATION POLICY (Events/Classes)

I understand all events are to be paid in full prior to the date of the event and that all classes and events do not honor a cancellation policy unless it is due to the venue cancelling or rescheduling the event due to not meeting minimum attendance requirements.
